Cypress HomeCare Solutions

The Personal Care Attendants can provide, but are not limited to the following:

1. Assists Client/Patient with personal care; bathing, dressing and grooming.
2. Assist Client/Patient with simple transfers such as in and out of bed or chairs.
3. Upholds a clean, neat and safe environment.
4. Assist with reminding client to take medication.
5. Psychological support and instruction for the families on practicing healthy eating habits.
6. Tuning client in bed to avoid bed sores.
7. Providing the Client emotional support.
8. Maintains current CPR and annual TB testing per policy.
9. Adheres to the Policies and Procedures established by Cypress HomeCare Solutions.
10. Adheres to the standards established by the Occupational Safety and Health Administration (O.S.H.A.)
11. Upholds safety by maintaining current CPR and First Aid Certificate.

Basic competency prior to scheduling clients will include but not limited to the following areas:

1. Basic characteristics of the physically impaired and older adults.
2. Recognition of significant physical, emotional and mental changes in Client/Patients condition.
3. Be fully trained to follow procedures for reporting changes to supervisor.

Personal Care Attendants will receive the same orientation certification program as Concierges and the same supervision as Concierges.

The Companions can provide, but are not limited to the following:

1. Assists with activities such as reading, conversation, crafts, needlework, games, movies, TV viewing, and other physical activities as desired.
2. Prepares meals, buys groceries, plans snacks and meals, and follows low-fat and low-salt meal plans.
3. Assists Person to and from appointments, lunches, dinners, movies and shows.
4. Performs home management skills including light cleaning, changing linens, laundry, washing dishes, trash removal and watering plants.
5. Sustains confidentiality with regard to the Client/Patient and adheres to the standards established by Cypress HomeCare Solutions.
6. Maintains current CPR and annual TB testing per policy.
7. Adheres to the standards established by the Occupational Safety and Health Administration (O.S.H.A.)
8. Upholds a clean, neat and safe environment.
9. Assist Client/Patient with simple transfers such as in and out of bed and chairs.
10. Companion services may NOT include heavy indoor cleaning of ceilings, walls and floors.

3. Be fully trained to follow procedures for reporting changes to supervisor.

The Concierges can provide, but are not limited to the following:

1. Assists Person to and from appointments, lunches, dinners, movies or shows.
2. Performs home management skills including changing linens, laundry, ironing, and washing dishes.
3. Provides additional house hold chores such as vacuuming, dusting, cleaning the kitchen and bathroom, trash removal, and watering plants.
4. Helps maintain a clean and safe environment for safety.
5. Assists with personal needs if requested such as dressing, prepares meals, buys groceries, plans meals and snacks, follows low-fat and low-salt meal plans.
6. Assist Client/Patient with simple transfers such as in and out of bed or chairs.
7. Assist with reminding client to take medication.
8. Assists with activities such as reading, conversation, crafts, needlework, games, movie, TV viewing, and other physical activities as desired.
9. Maintains confidentiality with regard to the Client/Patient medical history and records.
10. Adheres to the standards established by the Occupational Safety and Health Administration (O.S.H.A.)
11. Adheres to the Policies and Procedures established by Cypress HomeCare Solutions.
12. Maintain safety by maintaining current CPR and First aid certificate.
13. Concierges services may NOT include heavy indoor cleaning of ceilings, walls and floors.

Basic competency prior to scheduling clients will include but not be limited to the following areas:

1. Caregivers must be able to recognize significant physical, emotional and mental changes in client's condition.
2. Understand basic characteristics of elderly and the physically impaired.
3. Be fully trained to follow procedures for reporting changes to supervisor.

Concierges will receive the same orientation certification program as Personal Care Attendants and the same supervision as Personal Care Attendants.

*Please Note: The Concierge will dress in a professional manner that will make the client feel at ease while out in the public.

The Housekeepers can provide, but are not limited to the following:

1. Performs home management skills including laundry, ironing, washing dishes, vacuuming, dusting, cleaning the kitchen and bathroom, changing linen, trash removal, and watering plants.
2. Prepares meals, buys groceries, plans meals and snacks, follows low-fat and low-salt meal plans.
3. Assists Client/Patient in diversional activities such as reading, conversation, crafts, needlework, games, movie and TV viewing, physical activities as tolerated.
4. Maintains a clean and safe environment.
5. Maintains confidentiality with regard to the Client/Patient and employees.
6. Adheres to the standards established by the Occupational Safety and Health Administration (O.S.H.A.).
7. Adheres to the Policies and Procedures established by Cypress HomeCare Solutions.
8. Maintains current CPR and annual TB testing per policy.
9. Homemaker services may include heavy indoor cleaning of ceilings, walls and floors.
10. Homemaker services may include cleaning of a yard and hauling debris or firewood.

Basic competency prior to scheduling clients will include but not be limited to the following areas:

1. Basic characteristics of the physically impaired and elderly.
2. Recognition of significant physical, emotional and mental changes in client's condition.
3. Procedure for reporting changes to supervisor.

Homemakers will receive the same orientation certification program as personal care attendants.

The Live-In Caregivers can provide, but are not limited to the following:

1. Assists Person to and from appointments, lunches, dinners, movies or shows.
2. Assists Client/Patient with simple transfers such as in and out of bed or chairs.
3. Prepares meals, buys groceries and/or plans meals and snacks.
4. Assist with reminding Client/Patient to take medication.
5. Around the clock companionship.
6. Social support such as playing cards, reading out loud and conversing.
7. Assists Client in activities such as crafts, needlework, games, movie and TV viewing, physical activities as desired.
8. Upholds a clean, neat and safe environment.
9. Sustains confidentiality with regard to the Client/Patient and adheres to the standards established by Cypress HomeCare Solutions.
10. Maintains current CPR and annual TB testing per policy.
11. Adheres to the standards established by the Occupational Safety and Health Administration (O.S.H.A.)

1. Provide patient care to assigned patient (s), under the direction of a registered or licensed practical nurse.
2. Maintain accurate and complete records regarding patient care.
3. Cooperate with health care personnel and/or the family to provide continuity of patient care.
4. Adhere to policies and procedures of Cypress.
5. Perform patient care services within the guidelines of the Arizona Nursing Assistant Consortium on Practice.
6. Show evidence of continuing education.
7. Demonstrates the ability to complete assigned tasks and ability to cooperate with other health care team members.
8. Arrives promptly at assigned facility or client's residence, allowing time to become familiar with the surroundings.
9. Maintains accurate use of the caregiver sheet.
10. Reports to the Field Supervisor any changes in Client/Patient needs or environment.
11. Maintains confidentiality with regard to the Client/Patient and employees.
12. Adheres to the standards established by the Occupational Safety and Health Administration (O.S.H.A.).Adheres to the Policies and Procedures established by Cypress HomeCare Solutions.
13. Maintain current CPR in Infant and Child level and annual TB testing per policy.

The Licensed Practical Nurses Can Provide, But Are Not Limited to the Following:

1. Provide skilled nursing care to the assigned patient(s) under the direction of appropriate clinical supervision.
2. Maintain accurate, complete records regarding patient care including charts, nursing care plans, and evaluation as a regular part of patient care.
3. Cooperate with health care personnel and/or the family to provide continuity of patient care.
4. Adhere to policies and procedures of Cypress.
5. Adhere to current health practices in evaluation, observation, and application of nursing skills.
6. Perform nursing care within the guidelines of the Arizona State Board of Nursing.
7. Performance of nursing care will be limited to the population in which you are trained and proficient.
8. Show evidence of continuing education.
9. Arrives promptly at assigned facility or client's residence, allowing time to become familiar with the surroundings.
10. Maintains confidentiality with regard to the Client/Patient and employees.
11. Adheres to the standards established by the Occupational Safety and Health Administration (O.S.H.A.).

The Registered Nurses can provide, but are not limited to the following:

1. Provide skilled nursing care to the assigned patient(s) under the direction of appropriate clinical supervision.
2. Maintain accurate, complete records regarding patient care including charts, nursing care plans, and evaluation as a regular part of patient care.
3. Initiate and apply appropriate preventative and rehabilitative nursing procedures and techniques and maintain communication regarding patient care with the physician and nursing supervisor.
4. Perform therapeutic measure prescribed by the physician.
5. Supervise ancillary personnel when assigned.
6. Cooperate with health care personnel and/or the family to provide continuity of patient care.
7. Adhere to policies and procedures of Cypress.
8. Adhere to current health practices in evaluation, observation, and application of nursing skills.
9. Perform nursing care within the guidelines of the Arizona State Board of Nursing.
10. Performance of nursing care will be limited to the population in which you are trained and proficient.
11. Show evidence of continuing education.
12. Arrives promptly at assigned facility or client's residence, allowing time to become familiar with the surroundings.
13. Maintains confidentiality with regard to the Client/Patient and employees.
