Cypress HomeCare Solutions Partners with Hospice of the Valley for CMS GUIDE Model to Improve Dementia and Alzheimer’s Care
Cypress HomeCare Solutions, (Cypress), a legacy provider of home care services, is excited to announce its partnership with Hospice of the Valley in being awarded the Centers for Medicare and Medicaid Services (CMS) GUIDE model, known as Guiding an Improved Dementia Experience program. This collaboration will ensure the delivery of high-quality, patient-centered dementia and Alzheimer’s care under the innovative CMS GUIDE model.
This partnership brings together Cypress, with 30 years of experience serving the community, Hospice of the Valley, with 47 years of expertise, and Home Instead, another highly respected home care provider with 30 years’ experience. The goal is to elevate care for tens of thousands of people living with all types and stages of dementia and to support their caregivers.

The CMS GUIDE model is a voluntary nationwide initiative supporting individuals living with dementia and their caregivers. Nearly 200,000 older adults in Arizona are living with dementia, and over 257,000 Arizonans are providing care to loved ones experiencing cognitive changes.
The GUIDE model focuses on interdisciplinary, ongoing care and support for those living with dementia.
