Their engaging conversation highlighted the extensive programs and services FSL offers to support seniors and their caregivers throughout Arizona.

FSL’s Comprehensive Services
Dr. Moore, who joined FSL a little over a year ago with an impressive background in public health, shared insights about the organization’s three main pillars of service:
Health and Wellness
FSL operates adult day health centers in Tempe and Glendale that follow a medical model with nursing staff on-site. These centers provide:
- Daily living activities assistance
- Nutritious meals
- Social activities
- Medical supervision
- Respite for caregivers
The centers are open Monday through Friday from 7:30 AM to 5:30 PM, offering a safe, engaging environment for seniors who need support during the day.
Connection Services
FSL addresses isolation and mobility challenges through:
- Transportation services: FSL helps seniors get to medical appointments, grocery stores, and other essential locations.
- Friendly callers program: For seniors living alone, regular check-in calls provide emotional support and connection.
- Home-delivered meals: Beyond nutritional support, these deliveries serve as wellness checks for isolated seniors.
- Senior centers: Located in Wickenburg and Peoria, these centers focus on socialization and preventing isolation.
- Community Action Programs (CAP): Providing assistance with rent, mortgage, and utility payments.
Housing Solutions
FSL maintains affordable housing throughout Arizona, including:
- Single-family homes
- Affordable housing complexes
- A pilot program placing community health workers in affordable housing complexes
As Dr. Moore noted, “We know that people want to be as independent as they can for as long as they can…when they have that community where they feel like they can have all of their social determinants of health needs met, that’s where people want to be, and that’s where they thrive.”
Innovative Health Programs
One of FSL’s most impressive achievements is their transitional care service in partnership with Dignity Health hospitals. This program:
- Follows up with patients after hospital discharge
- Works to prevent hospital readmissions
- Has achieved a remarkable 3% readmission rate compared to the national average of 18%
FSL also operates 17 assisted group living homes for individuals with serious mental illness diagnoses who need daily living assistance, helping prevent institutionalization while supporting independence.
Caregiver Support
FSL recognizes the vital role caregivers play and offers specialized support:
- The Caregiver House: A fully ADA-compliant home designed to train caregivers on safely assisting loved ones
- Resources, Information, Support and Education (RISE) team: Provides education on fraud prevention, nutrition, fitness, and other essential topics
- Home improvement services: Professional installation of grab bars, benches, and other safety features to prevent falls
Looking Forward
After celebrating 50 years of service in 2023, FSL is undergoing rebranding to better reflect their comprehensive services that extend beyond senior care. The new name will be revealed on April 21, 2025.
